No two people are the same, so why should your

That’s why we’re supporting a patient-centered
approach that puts you at the center of your health
care. That means focusing on preventing health
problems in the first place. How? We’re giving
PCPs an even more important role through
patient-centered care. And we’re giving them
the right information, tools and support so
they can manage your care based on your
own health history.
We hope to help improve your health and care
and lower your costs through:

Better access:
You’ll be able to go to your PCP
when you need to – after hours, on the weekends
or even on the web. And your PCP will have the
time needed to give quality care.

Lowering health risks:
That means your PCP
will have information that can identify when you
haven’t had recommended treatments. Your PCP
will be able to take care of any gaps in care (like
a missed eye exam) and help keep you out of the
hospital or emergency room (ER) more often.

Coordinating care:
PCPs will have the tools
and information they need to give the best care.
They’ll be able to handle more of your care,
